Microchip
ISO 11784/11785 microchip, implanted on or before the first rabies vaccination.
Rabies vaccination
Two vaccinations, not less than 30 days and not more than 365 days apart, administered once the pet is resident in Poland. Australia only recognises vaccinations given in a DAFF-approved country, so a course started in Belarus does not count toward the Australian requirement.
Quarantine
Minimum 10 days at Mickleham Post Entry Quarantine Facility, Melbourne, with identity verification completed beforehand; 30 days minimum without it. Weekday arrivals only. Melbourne is the only port that accepts pet imports.
Import permit
DAFF import permit applied for via BICON during the Polish residency period.
Health certificate
DAFF-format health certificate, issued in Poland within 5 days of departure, not in Belarus.
Export permit (Belarus)
